Payment and Insurance for Office Visits:
Payment is appreciated and due at the time of the service.
There is a consultation fee for the initial visit. Procedures and
tests performed during office visits incur separate additional charges,
as do medications and/or skin care products.
We accept cash, personal checks, American Express, Visa, Discover
and MasterCard. We will give you a Universal Claim form to submit
to your insurance company for reimbursement for non-cosmetic services.
We have structured our office hours to facilitate in-office cosmetic
procedures and treatments, and we regret that therefore you will
be billed for any missed/cancelled appointments without 24 hours
notice.
